Achilles tendon debridement - operative indications

Tendonitis or tendonosis which has failed to settle by conservative means (Ie Physiotherapy, activity modification, Aircast boot, injection).

If extensive and severe degenerative change is present consideration should also be given to a more radical debridement and filling the defect created with a Flexor Hallucis Longus transfer.

Generally a posteromedial scar is used, which avoids direct rubbing of the scar on shoe wear and allows easy access through the same scar for a Flexor Hallucis Longus transfer should this be later required.
