1-Tendo Achilles
2-flexor hallucis longus muscle belly, seen after dividing the overlying fascia
3-posterior tibial nerve (if one approaches it beneath the fascia, just medial to FHL muscle (as here) it will be deep to the vessels which will therefore not be seen (Compare with the view seen during posterior ankle decompression). If one dissects onto it through the fat from superficial to deep, one will need to deal with the vessels.
