A medial portal is established in the space between the Tibialis Anterior tendon(2) and medial malleolus(1) and the anterior lip of the tibia superiorly. This is best identified by plantar and dorsiflexing the ankle and observing the opint at which movement occurs, beneath the unmoving tibia. This will be the joint line. The "sweet spot " for injection and portal placement.It does get easier.
