The antero-lateral portal formed using the same technique with initial skin incision. The Superficial peroneal nerve is at risk here so avoid plunging the blade deeply.
Flexing the fourth toe can sometimes make the nerve more visible(prior to setting up in traction) but this is not a particularly reliable physical sign.
