The capsule is divided in the line of the skin incision, producing superior (3) and inferior (2) flaps. The lateral talus is exposed. Often the anterior Talo-fibular ligament is not discreetly identifiable following injury. Occasionally it is and may then be re-attached. The dissection is carried posteriorly in the line of the arrow back to the peroneal tendons (4).
