The osteotomy is performed using a large one sided reciprocating saw blade (1). The osteotomy location is between the achilles insertion (2, which should be identified) and posterior facet superiorly and exits inferiorly distal to the weight bearing surface (3). At this level the neurovascular bundle should sit anteriorly and so away from the blade. Two large Trethowans retractors are placed superiorly and inferiorly at the point of the cut, their tips carefully placed on the medial aspect of the calcaneum.
