Using a De Soutter high speed 5mm burr (1) the base of the proximal phalynx is taken back to healthy subchondral bone. The surface should be left concave to allow congruent positioning with the convex metatarsal head. Care should be exercised at the most plantar aspect to avoid damage to the flexor apparatus (2).
