The FHL tendon(1) lies deep to the neurovascular bundle(2). The tendon sheath investing the FHL has already been partially divided to allow this view. Vascular branches run anteriorly (3) and will need to be diathermied or ligated to gain sufficient access to the tendon and posterior aspect of the ankle. The bundle is being gently retracted using vascular sloops(4).
