The Ilizarov frame is fixed to the lower tibia and foot in its deformed position. The posterior soft tissue contracture is corrected by progressively distracting the posterior aspect of the ankle(1) by shortening of the anterior soft tissues(2) .This is performed by shortening the anterior struts(b).It is important that the hinge(a) which slowly rotates during this process is placed at the centre of rotation of the ankle joint ,otherwise anterior subluxation of the ankle will also occur. A hinge must also be placed between the bottom of the "motor struts"(b) and the front of the foot frame.
