Immediately deep to the Tibialis posterior tendon (1) is a tough fibrous layer, the Spring Ligament. This has been subperiosteally reflected to expose the medial wall talus (2) and subtalar joint (3). An osteotome is placed at the guessed level of the joint (4). Its position is checked with image intensification.
