The diagnosis of tarsal tunnel syndrome is often made clinically with characteristic timing and distribution of symptoms.

Nerve conduction tests may confirm the diagnosis but negative test are not uncommon with this condition and need not prevent treatment.

Mri or ultrasound can reveal extrinsic compressive lesions neighbouring the posterior tibial nerve and its branches.
