A subperiosteal dissection is needed to expose the proximal half of the proximal phalynx. The flexor mechanism needs to be carefully freed to allow a homans retractor plantarwards , as well as dorsally(3).The capsular attachments to the base of the proximal phalynx (1,4) need to be left attached if a capsular correction for the MTP is required(as here with a scarf osteotomy,(2).
