Once the skin incision is made for the scarf osteotomy gently go through the fat in the longitudinal line of the skin incision with fine tenotomy scissors(not shown). There is always another layer superficial to the capsular layer which in particular needs to be identified dorsally and then defined with a fine blade as shown. The dorsomedial cutaneous nerve(2) sits here and should be reflected dorsally to avoid injury.
