Scarf Osteotomy - the Barouk screw
The planned screw length is measured from the guide wire. Next subtract 2-4mm from the measured length to avoid joint penetration by the screw then over drill with long then short (to counter sink screw head) drills, the Barouk screw is inserted (1) to fix the distal end of the scarf osteotomy. Previous countersinking will ensure that the head is not prominent. By careful placement of the guide wire (not too far distal or lateral) one has ensured that an intact shelf of dorsal bone remains distally and laterally.
